Insider Fraud in Accounts Payable: Detection Before the Money Leaves

Insider fraud is the hardest category of AP fraud to prevent and detect. External attacks require the attacker to impersonate, intercept, or deceive. Insider fraud is committed by someone who already has legitimate access — and uses that access to steal.

According to the ACFE's 2024 Report to the Nations, insider fraud schemes cause median losses of $150,000 per incident and take an average of 12 months to detect. In AP specifically, where employees have direct access to payment systems, vendor records, and approval workflows, insider fraud can operate undetected for years.

How Insider Fraud Happens in AP

Ghost vendor creation. An AP employee with access to the vendor master creates a fictitious vendor — often using a personal bank account or a shell entity — and submits invoices for services never rendered. Because the same person controls both the vendor record and the invoice entry, standard workflow approvals can be manipulated or bypassed.

Vendor master modification for personal gain. An employee with access to vendor banking details changes the account for a legitimate vendor to one they control, then processes payments that are meant for the real vendor. The real vendor doesn't receive payment; the employee pockets the difference. The fraud is discovered when the vendor calls about unpaid invoices.

Fictitious employee payroll. In organizations where the same person manages both HR record creation and payroll processing, fictitious employees can be added to payroll and paid without independent verification. This is one of the most common insider fraud schemes in smaller organizations.

Overpayment and rebate theft. An AP employee deliberately overpays a vendor and arranges for the vendor to return the overpayment — directly to the employee, rather than to the company. This requires vendor collusion but is documented in multiple high-profile cases.

Expense reimbursement fraud. Employees submit personal expenses — including personal purchases, inflated amounts, or entirely fabricated receipts — for reimbursement through the AP system. In organizations without systematic receipt verification, small recurring fraudulent expense claims can accumulate to significant losses over time.

Why Insider Fraud Is Hard to Catch

Access is legitimate. Insider fraudsters use their real credentials to make real changes in real systems. There's no external attack signature to detect.

Transactions look normal. A ghost vendor invoice that an AP employee created looks exactly like a legitimate invoice in the system — because the same person controls both the vendor record and the invoice.

Segregation of duties is often absent. The most effective control against insider fraud — ensuring that no single person can both set up a vendor and approve payment to that vendor — requires staffing levels and process discipline that many organizations don't maintain.

Duration before detection is long. Because insider fraud blends into legitimate payment flows, it often runs for months or years before being caught — typically by accident (an employee's departure, an external audit, a tip from a colleague) rather than by a control.
